Leslie Mikkelsen, RD, MPH
Leslie Mikkelsen, RD, MPH, Managing Director of Prevention Institute directs the Health Systems Transformation team. The team focuses on promoting community-level prevention in the implementation of the Affordable Care Act and fostering healthcare – community integration practices and policies. Current efforts underway include technical assistance for funding initiatives in various U.S. regions that support community and rural health centers in adopting Community-Centered Health Homes practices, a model developed by Prevention Institute. A concentration of the team is analysis of how to effectively incorporate community-wide prevention in Accountable Communities for Health. Leslie served on the California Accountable Community for Health Advisory Committee which informed the California State Innovation Plan. Leslie provides support to the Convergence Partnership—a consortium of national funders—with strategy development and implementation to promote their vision of Healthy People, Healthy Places. She co-founded and served as Project Director for the Strategic Alliance for Healthy Food and Activity Environments, a California coalition that advances a change agenda influencing state policy. She previously worked for the Alameda County Community Food Bank in Oakland, CA and Food For Survival Food Bank in New York, NY. She is a registered dietitian and a graduate of the University of California, Berkeley where she earned her Master of Public Health.
