Please join us for a panel discussion about diabetes with nationally renowned experts, Drs. James Gavin, Ann Albright, and George Rust, who will lead the conversation about barriers to integrated clinical care and practice transformation, community engagement, community partnerships, and data collection and analysis. We will also examine ways health professionals are successfully working together to make a difference.

This Web Forum will specifically address:

  • Diabetes burden for minorities in the United States;
  • Causes of health disparities;
  • Interventions needed to achieve diabetes health equity;
  • Ways to achieve tighter integration of clinic and community interventions; and
  • How research is translating into better outcomes for diabetes patients.

This activity has been reviewed and is acceptable for up to one credit by the American Academy of Family Physicians.
