Dr. Jaffe is the Clinical Leader of the Kaiser Permanente Northern California Cardiovascular Risk Reduction (PHASE) Program, and has served in this position since the program was established in 2004. Dr. Jaffe also serves as the Clinical Leader of the Care Management Institute (CMI) Kaiser National Integrated Cardiovascular Health Guideline Team. Dr. Jaffe joined Kaiser in 1993 and practices Endocrinology and Internal Medicine at the Kaiser South San Francisco Medical Center. He is currently the Chief of Endocrinology at the Kaiser South San Francisco Medical Center, and has previously served as Chair of the Kaiser Northern California Chiefs of Endocrinology, the clinical leader for Kaiser CMI National Hypertension Guideline team, and the Kaiser Northern California Hypertension guideline team. He clinical research area is the delivery of population based cardiovascular care and he has published many papers on this subject. He is an Associate Clinical Professor of Medicine at the UCSF Medical School and an attending physician at the San Francisco General Hospital General Endocrinology Teaching Clinic. He is board certified in Internal Medicine and in Diabetes, Endocrinology, and Metabolism.
