Please join Nemours Children’s Health System for an engaging discussion on community care coordination systems and the role of technology applications that are emerging to “close the loop” between clinical and community services.

The health care system is transforming to deliver value and maximize health across the lifespan. In this new landscape, there is rapid adoption of health technology to deliver patient-centered, whole-person care approaches. As states, communities, health systems and providers begin to address the social determinants of health, an essential element is a comprehensive system that connects patients with both the health and social service sectors so all needs are addressed.

In this web forum, presenters will share perspectives from stakeholder interviews on challenges and accelerators, highlight findings from a survey of technology vendors, and provide recommendations for policymakers, community leaders, and public and private funders.

Registration is free and closed captioning is available to all attendees. This event is recommended for anyone working in public health, advocacy, community-based systems, hospitals/health systems, insurers, and funders.
