The Evolution of the CHW Hub within Public Health

Riverside County has embarked on an exciting journey of establishing a Community Health Worker (CHW) hub that focuses on a centralized process for CHW training, deployment, referrals, intake, billing and claims management. Come learn how Riverside University Health System – Public Health has created the CHW Hub to serve the community in a health system that includes public health, hospital, and outpatient clinical setting partners.

Learning Objectives:

  • Learn how to build and sustain a revenue generating program

  • Learn how to leverage relationships with Community based organizations (CBOs) and other partners for successful recruitment efforts

  • Understand the importance of building relationships with local Managed Care Plans (MCPs) for assistance with program development
