Annie Pham

Director, Social Health, Community Care Cooperative


Annie Pham is a health care professional committed to improving the health and well-being of under-resourced communities. She is passionate about health equity, and as the Director of Social Health at Community Care Cooperative (C3), an accountable care organization in Boston, MA, she develops and executes projects to address health related social needs for patients at federally qualified health centers. As part of a Medicaid Section 1115 Demonstration project in Massachusetts, she developed a portfolio of clinical-community partnerships and evidence-based nutrition and housing interventions to serve over 13,000 members in the State. Her work also entails capacity building and workforce development initiatives to equip health center and social service staff with the tools and knowledge required to identify and address unmet health related social needs.

Annie earned a Bachelor of Science and Master of Health Services Administration (MHSA) from Xavier University in Cincinnati, Ohio. She is a graduate of the CDC Public Health Scholars Program, where she received training on the fundamentals of public health, minority health, and health disparities. She is a co-author of the publication “Applying Value Chain Thinking to Social Drivers of Health: A Framework and Two Case Studies,” which appeared in the December 2021 issue of the New England Journal of Medicine Catalyst. She currently resides in her hometown of Worcester, Massachusetts, and enjoys being close to friends and family.
